We are seeking a passionate and dedicated Case Manager Intern to join our team! This internship offers a valuable opportunity to gain hands-on experience in case management, social services, and nonprofit program delivery, making a direct impact on the lives of youth and families in need.
Position Overview
As a Case Manager Intern, you will be an integral part of our programs team, assisting with the assessment, support, and advocacy of program participants. You will be involved in connecting individuals to our programs, conducting intake interviews, making referrals to community partners, and providing follow-up support.
Key Responsibilities
Conduct Initial Intake Interviews:
- Engage with participants to assess their needs, goals, and eligibility for programs.
- Document participant information accurately and maintain confidentiality.
Connect Participants to Programs and Services:
- Match participants with appropriate Motivated and Empowered Inc. programs, such as after-school support, financial literacy, mentorship, and more.
- Make referrals to external community partners for additional support, including mental health services, housing, and counseling.
Counseling and Support Placement:
- Assist with identifying counseling resources and facilitate placement based on participant needs.
- Maintain ongoing communication with counseling partners and participants to ensure seamless service delivery.
Ongoing Case Management and Advocacy:
- Provide consistent follow-ups to assess participant progress and address challenges.
- Advocate for participants, ensuring they receive the necessary tools and support to achieve their personal and educational goals.
Resource Coordination and Participant Engagement:
- Collaborate with the team to organize and coordinate resources, including educational workshops, health and wellness events, and community outreach programs.
- Support participant engagement through workshops, group sessions, and individualized support.
Administrative Support:
- Update participant records, track progress, and maintain accurate documentation.
- Contribute to program evaluations and feedback to enhance services and participant outcomes.
Qualifications
- Currently pursuing or recently completed a degree in Social Work, Counseling, Psychology, Human Services, or a related field.
- Strong interpersonal and communication skills with the ability to relate to diverse populations.
- Passion for working with youth and families and a desire to make a positive impact.
- Organized, detail-oriented, and capable of managing multiple tasks simultaneously.
- Ability to maintain confidentiality and demonstrate ethical behavior in all interactions.
- Basic understanding of case management or social services (preferred but not required).
- Bilingual abilities are a plus but not required.
